Your Opportunity
The Psychosocial Support Service aims to strengthen the capacity of consumers to live independently, safely, and productively in their community, form meaningful connections in a supportive environment, and reduce the need for acute care.
This role will support consumers to access and maintain engagement with the clinical supports and to establish meaningful personal and community connections and connect them to the support needed to develop their skills to sustain housing and live successfully in the community.
Your key responsibilities will be to:
- Conduct comprehensive assessments to identify psychosocial needs of the clients and develop individualized case plans
- Manage caseload of clients, create holistic strengthen based support plans and monitor progress
- Provide an intensive wrap around support with referral and connection to services
- Manage incoming new referrals of clients
- Develop and maintain relationships with internal and external stakeholders
- Provide accurate record keeping and updating of case notes and reports
- Uphold the program values and standards to ensure a positive outcome
Requirements for Success
- A relevant qualification in Community Services, Social Work or related field
- Strong Case management experience
- Driverโs Licence
- Trauma informed knowledge
- Senior First Aid Certificate (or willing to obtain)
- Excellent interpersonal skills with ability to work cooperatively in a team environment and independently
- Excellent internal and external stakeholder management with high level communication skill
