Your Opportunity
The Grove Western Sydney youth program is a specialist homelessness service which provides support to young people between the age of 16-25 who are experiencing homelessness or at risk of homelessness.
This role will provide holistic personalized case management and housing support to young people in and around Blacktown LGA and the Hills district. Outreach rough sleeper patrols are a key component for the team, requiring an early morning start on a rostered basis.
This is a maternity cover position ending on 30th June 2025.
Your key responsibilities will be to:
- Undertake initial program intake assessment and provide ongoing case management support
- Work with other services to create individualized case plans and assist the clients to engage in goal setting
- Provide follow-up support to ensure clients who have been housed sustain their tenancies
- Develop strong relationships with key internal and external stakeholders to effective functioning of the service and improved outcomes
- Complete a range of administrative duties including client files, case notes, referral/support letters and support plans and monthly reporting
Requirements for Success
- Minimum of a Cert IV in community services/youth
- Minimum 2 years’ experience in working with at-risk youth
- Strong communication, empathy and advocacy skills
- Have a sound understanding of Mission Australia’s values and code of conduct
- Well-developed written and oral communication skills, including highly developed administrative skills
Before starting work with us, you will need to undertake a national police history check, qualifications, referee checks, and WWCC.
