Your Opportunity

Working across Northern Sydney and Northern Beaches, you will provide case management support to people with AOD use or misuse and complex psychosocial need to be able to achieve their goals

Your key responsibilities will be to:

  • Face to face case management support for people with AOD use or misuse
  • Provide an intensive wrap around support with referral and connection to services
  • Manage incoming new referrals of clients
  • Develop and maintain relationships with internal and external stakeholders
  • Provide accurate record keeping and updating of case notes and reports
  • Uphold the program values and standards to ensure a positive outcome

Requirements for Success

  • A relevant qualification in Community Services, Social Work or related field
  • Strong Case management experience
  • Driver's Licence
  • Trauma informed knowledge
  • Senior First Aid Certificate (or willing to obtain)
  • Excellent interpersonal skills with ability to work cooperatively in a team environment and independently
  • Excellent internal and external stakeholder management with high level communication skill
